Mayer Veterinary Hospital makes it easy to ensure your pet is taken care of. We provide online forms for your convenience. If you don’t already have AdobeReader installed, you’ll need it to complete and send these forms.

You can fax the forms or bring it with you to your appointment. Every first-time patient needs a New Patient Registration form to record important data about your four-legged friend into our software program.

If you’re boarding your pet, you’ll need to complete the Boarding Form. For those patients that require surgery, you’ll need to fill out and sign the Anesthesia Consent Form.

Payment

Payment is expected when serviced are rendered. Mayer Veterinary Hospital doesn’t send out bills. We accept cash, checks, and debit and credit cards. All cards used must be signed by the card owner.

You can call our clinic at 337-988-3303 to schedule an appointment for your pet to be seen around your schedule. You should have a vet look at your pet for an annual checkup. This visit is a full exam and may include teeth cleaning if needed.

To help ensure your pet receives the best care, bring in any medical history of your pet. If you have a journal about important information on your furry friend, bring it along. Note anything that is unusual or out of character for your pet. Dr. Bill Mayer will check over your pet and their various systems to ensure they are in good condition. If Dr. Mayer notices anything of concern, he will discuss the options with you. In certain situations, he may refer you to a specialist who has more expertise in specific areas.
